The Puerto Rico Judicial Power website has several forms for restraining orders:

  • In this link, you will find the form to file for a domestic violence protection order, in English and Spanish, and here you can fill out the paperwork online, print it and bring to the courthouse;
  • Here you can find the form for a sexual violence restraining order and the personal information form for this petition, only available in Spanish; and
  • Here you can find the form for a restraining order for stalking victims and the personal information form for this petition, only available in Spanish.
