Kentucky Court Forms

The Kentucky Courts website lists many forms online for a variety of different court proceedings. Click on “Protective Orders - Domestic Violence / Interpersonal Violence” in the left-hand menu to see the protective order forms they have available. For custody proceedings involving relocation of a child, you can download the notice of relocation that must be sent to the other party. For small claims court actions, you can find the forms for the summons, complaint, and counter-claim.
