Arizona Court Forms

Updated:
October 15, 2020

The Arizona Supreme Court’s Self-Service Center website has a tool that helps you fill out protective order forms by answering a series of questions. They also list additional forms for topics such as appeals, child support, small claims court and more.

You may also find court forms in the clerk’s office at the courthouse. See our AZ Courthouse Locations page. You may also be able to obtain them by calling a local domestic violence organization or legal assistance provider. You can go to our Places that Help page for referrals.
